Fees and Insurance
I accept several major insurance plans and Medicare. As an In-Network insurance provider, my office will bill your insurance as a courtesy and your co-payment, as specified by your insurance plan, will be expected to be paid at each visit. Please call to find out if I am in your plan. If I am not in your insurance plan, check with your provider, as I am also an out of network insurance provider recognized by various insurance companies. As an Out of network provider, payment for individual and couple’s sessions are due at each visit. I will provide you with an invoice which is suitable to submit to your insurance company for reimbursement of fees paid. My fee is $180.00 and sessions are 45 minutes each. Check, cash or credit card are accepted for payment.
Reduced fee services are available on a limited basis. Please call to discuss your circumstances.
24-hour notice is required to cancel or reschedule appointments. Without this notice, missed appointments are charged in full and you will be billed for the session. Of course, emergencies and illness are considered.
